Perforated septums have many causes and the etiology should be determined by a head and neck (ENT-Otolaryngology) surgeon; they can be a sign of serious conditions which may need to be ruled out.
Overall, septal perforations can occur as a result of previous nasal surgery, or as a result of a nasal fracture. A septal perforation is also often thought of as resulting from cocaine use, but various forms of trauma are also common causes. The photo below is an example of a perforation of the nasal septum.

Unrepaired perforations can get larger, cause whistling, crusting, nasal obstruction, and bleeding and may be chronically infected. Some can cause collapse of the external nose. The photo below is an interoperative photo of a Septal Perforation before and after repair.
